Description
Healthcare partnerships are one of the fastest growing sources of revenue for AAAs and other CBOs that support older adults in their homes and communities. At the same time, healthcare entities are more interested than ever in working with CBOs to address social determinants of health (SDoH) and to improve healthcare outcomes. CBOs have greater opportunity to engage with healthcare partners, yet the pathway to a formal relationship is not always straightforward or easy. This webinar highlights some of the key issues for CBOs to be aware of when considering healthcare partnerships from assessing system, service, and financial readiness to HIPAA compliance to measuring data on outcomes.
Topics Discussed:
- Assessing organizational readiness
- Partnership expectations from healthcare entities
- Building the data case: collection, tracking, and moving from outputs to outcomes
